4. Voters Affidavit (COMELEC-San Fernando)
5. CERTIFICATE OF REGISTRATION (OR/CR OF UNITS)
6. Franchise MTOP and Provisional authority Plus annual fee Php 100.00
7. POLICE CLEARANCE
8. MAYOR'S CLEARANCE
9. SSS Clearance
10. BIR Clearance
11. PHILHEALTH Clearance
